I didn't catch the 'mad' thing - I'm Australian, which means my vocabulary seems to sit in that weird place between British and American, and sometimes I'm not aware of those language issues, so thanks for pointing it out :)
As for Arthur calling him Uther, that was pretty much a specifically-talking-to-Morgana thing, since a) she calls him Uther, and b) given the strained relationship between her and Uther, it's not unforseeable that Arthur might want to distance himself from his father by not referring to him that way. Also, while there are only really spoilers up to The Beginning Of The End, I saw this as taking place post-season one, when it's conceivable that their relationship might have shifted to the point where he might call Uther by name.
... Which is probably more explanation than you asked for, sorry :P
